Independent Medical Examination Platform

Turn hours of IME chart review into minutes.

Aequitas drafts the Records Review, captures the exam with an ambient AI scribe, and synthesizes a defensible assessment — across the three parts of an IME report. The AI drafts; the physician edits and signs. Impartial by design, secure by default.

HIPAA-aligned · PHI encrypted at rest & in transit · BAA available

Part I

Records Review

Upload scanned records — OCR + AI draft a chronological, source-attributed review.

Part II

Examination

Ambient transcription structures the exam into findings, history, and Q&A.

Part III

Assessment

AI synthesizes diagnoses, causality, and work-capacity for your review.

The IME workload, not the IME judgment

A single Workers' Compensation file can run to hundreds of pages of scanned, out-of-order records. Physicians spend hours reconciling timelines, then more hours dictating and formatting a report — time that pulls against the independent judgment the exam is meant to deliver.

Aequitas removes the busywork

  • OCR and AI assemble a clean, chronological Records Review with citations back to the source page.
  • An ambient scribe captures the exam so you can focus on the claimant, not the keyboard.
  • A drafted assessment ties Parts I and II together — ready for your edits and signature.

How it works

One report, three guided parts

Aequitas mirrors the structure of a defensible IME report — and does the heavy lifting at each stage while keeping the physician in control.

Part I

Records intake & review

Upload any number of case records — most arrive as scanned PDFs. Aequitas OCRs every page automatically and Claude AI drafts a chronological, source-attributed Records Review, with each fact traceable back to its origin document.

  • Unlimited record uploads
  • Automatic OCR of scanned PDFs
  • Source-cited chronology
Part II

Ambient exam capture

During the physical examination, room audio is transcribed with clinically-tuned speech-to-text. The AI structures the conversation into examination findings, the claimant history, and a Q&A — so your attention stays on the exam.

  • Clinically-tuned transcription
  • Structured findings & history
  • Captured Q&A
Part III

Assessment & recommendations

Aequitas synthesizes Parts I and II into diagnoses, a causality opinion, a disability and work-capacity determination, and recommendations. You edit every section and sign. The opinion is always yours.

  • Diagnoses & causality opinion
  • Disability / work-capacity
  • Physician edits & signs

Features

Everything an IME report demands

Purpose-built for the realities of Workers' Compensation case files — and the standards a report has to meet.

OCR for scanned records

Every uploaded PDF is OCR’d automatically, so even faxed or photographed records become searchable, citable text.

Ambient AI exam scribe

Clinically-tuned speech-to-text captures the exam in the room and structures it into findings, history, and Q&A.

AI-drafted, physician-signed

The AI drafts each part; you edit every section and apply your signature. The medical opinion is always the physician’s.

Permissioned access for counsel

Grant claimant and defense attorneys secure, per-case access to specific files — nothing more, nothing less.

Complete audit trail

Every upload, edit, access grant, and signature is recorded in an append-only log you can stand behind.

DOCX export

Export the finished report to a clean, formatted Word document ready for filing, the carrier, or counsel.

Security & compliance

Built for PHI from the first line of code

IME work lives in sensitive medical and legal records. Security and compliance are not a feature here — they are the foundation. Here is how we protect every case file.

Request our security overview

Encrypted at rest & in transit

PHI is encrypted in transit with TLS and at rest using managed KMS keys. Case data is never left in the clear.

Least-privilege access

Every account and service runs with the minimum access required. No standing access to data that a role doesn’t need.

Per-case permissioning

Counsel and staff are granted access to specific cases and files — scoped, reviewable, and revocable at any time.

Append-only audit log

Access, edits, and signatures are written to an immutable, append-only trail that supports defensibility and review.

BAA available

We sign Business Associate Agreements. Our handling of PHI is designed to align with HIPAA obligations.

Physician stays in control

AI drafts; the physician edits and signs. No report is final until the examining physician approves it.

Aequitas is HIPAA-aligned and offers a BAA. We do not claim certifications we don’t hold; we’re happy to walk your security team through our controls.

FAQ

Questions, answered

Is my data secure?

Yes. PHI is encrypted in transit (TLS) and at rest with managed KMS keys, access follows least-privilege principles, and every action is recorded in an append-only audit log. Case access is permissioned per case and revocable.

Does the physician stay in control of the report?

Always. Aequitas drafts each part of the report with AI, but the examining physician edits every section and applies their signature. No report is final until the physician approves it — the medical opinion is theirs.

What file types can I upload?

Case records are typically scanned PDFs, including faxed or photographed documents. Aequitas OCRs every page automatically so the content becomes searchable and citable, regardless of how the records arrived.

How does counsel access work?

You grant claimant or defense attorneys secure access to specific cases and files — and nothing else. Access is scoped per case, fully logged in the audit trail, and can be revoked at any time.

Is Aequitas HIPAA compliant?

Aequitas is built to align with HIPAA obligations and we sign a Business Associate Agreement (BAA). We’re glad to review our controls with your security or compliance team. We do not claim certifications we do not hold.

How does the ambient exam scribe work?

During the physical examination, room audio is transcribed with clinically-tuned speech-to-text. The AI then structures the conversation into examination findings, the claimant history, and a Q&A — which you review and edit.

Can I export the finished report?

Yes. Completed reports export to a clean, formatted Word (DOCX) document ready for filing, the carrier, or counsel.
